Lexus UX 250h (2023): High-Voltage Battery Inspection
The UX 250h uses a nickel-metal hydride high-voltage battery located at the rear of the body, under the rear seat. The main check is done with a scan tool using hybrid system data, without disassembly: state of charge, block voltages, imbalance, and temperature. Opening the battery or working on the HV circuit is allowed only with the system shut down and with proper protection.

Step by step
10 steps- Preliminary symptom check: hybrid system malfunction indicator on, state-of-charge gauge dropping quickly, the engine starting often while parked, higher fuel consumption. Read the fault history and any saved freeze-frame data.
- Connect the scan tool to the OBD port under the dash, to the left of the steering column. Turn the vehicle on to READY mode and read the hybrid system and battery trouble codes (the P0A80 group: replace hybrid battery pack; P0AFA: low battery voltage; and related codes).
- In the hybrid battery data, evaluate: state of charge (SOC), voltage of each block/module, the difference between the highest and lowest block, internal resistance, and battery sensor temperatures. A noticeable block imbalance or abnormal resistance in one block indicates degradation. Acceptable limits for this model are not documented in public sources, so rely on the thresholds shown by the scan tool itself.
- Run an active test if your scan tool supports it: forced charging/discharging and operation of the battery cooling fan. Compare how the block voltages behave under load.
- Check the battery cooling system: the air intake near the rear seat must not be blocked by belongings or clogged with dust and pet hair, the filter must be clean, and the fan must start and run without unusual noise. Overheating is a common cause of accelerated degradation.
- Check the 12 V battery: a weak 12 V battery causes false hybrid system errors and starting problems. With the system off, voltage should be about 12.4-12.7 V.
- If a physical inspection or measurement at the battery itself is required: turn the vehicle off, move the key/smart key out of range, disconnect the negative terminal of the 12 V battery, remove the high-voltage service plug, and wait at least 10 minutes for the capacitors to discharge. Check the plug location and removal procedure against the UX service manual. Do not pry off trim panels blindly.
- Before touching any orange cables or terminals, confirm with a multimeter that there is no voltage. Work only in 1000 V insulating gloves, and take off rings and watches.
- Evaluate the result: with minor imbalance, reconditioning/balancing charge with specialized equipment may be possible; if one or two blocks have failed, replace the blocks or the whole battery assembly; if the battery is healthy, look for the cause in the inverter, current sensors, or the 12 V circuit.
- After the work, reinstall the service plug, reconnect the 12 V battery, clear the codes, and take a test drive, then read the battery data again.
Common mistakes and warnings
- The high-voltage system can be fatal: the orange cables and the battery carry high voltage
- If you lack experience with hybrids and insulating protective gear, limit yourself to scan tool diagnostics and leave opening and replacing the battery to a professional repair shop
- Tighten HV battery fasteners and terminals to the manufacturer's specified torque
- Do not tighten them by feel
